If you are visiting us for the first time, you will need to fill out some paperwork before you start your treatment. Please click here to print the forms needed for your first visit.
 

Before your treatment.. :
 

• Eat a regular meal
   

• Take your medications as regularly prescribed by your doctor
   

• In case you are required to take antibiotic premedication before your dental treatment, please premedicate one hour before your appointment. As well, be aware that the recommendations have recently changed regarding antibiotic premedication. 
   

• If your referring dentist has sent a referral slip and/or x-ray, please bring it to your appointment 
   

• Feel free to contact our office with any questions you may have about your insurance and/or financial arrangements or if you have any other concern
